Welcome to the website of Summit Mind Psychiatry, PLLC (“Summit Mind Psychiatry,” “we,” “our,” or “us”). By accessing or using this website, you acknowledge and agree to these Terms of Use. If you do not agree with these Terms, please discontinue use of the website.
These Terms apply to use of the Summit Mind Psychiatry public website. Separate policies, notices, consents, and agreements may apply when you become a patient or receive healthcare services from Summit Mind Psychiatry.
1. General Information Only
Information provided on this website is intended for general informational and educational purposes. Website content is not intended to provide individualized medical, psychiatric, psychological, diagnostic, treatment, medication, or other professional healthcare advice.
Do not disregard professional medical advice, delay seeking care, start or stop medication, or make treatment decisions solely because of information presented on this website.
2. No Provider-Patient Relationship
Accessing this website, reading its content, submitting an inquiry, sending an email, completing a website form, or requesting an appointment does not by itself establish a provider-patient relationship with Summit Mind Psychiatry or any of its healthcare professionals.
A provider-patient relationship is established only after applicable intake and consent requirements have been completed and Summit Mind Psychiatry has agreed to provide healthcare services.
Submitting an appointment request does not guarantee that an appointment or provider-patient relationship has been established.
3. Emergencies and Crisis Services
This website is not intended for emergencies or crisis services.
Summit Mind Psychiatry does not provide emergency or crisis response through this website, email, contact forms, or routine electronic messaging.
If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department.
If you are experiencing a suicidal, mental health, or substance use crisis, call or text 988 to reach the 988 Suicide & Crisis Lifeline.
Do not use website forms, email, appointment requests, or routine patient messages for emergency or time-sensitive safety concerns.
4. Healthcare Services
Information describing Summit Mind Psychiatry's services is provided for general informational purposes and does not guarantee that a particular service, treatment, medication, diagnosis, or course of care will be appropriate or available for any individual.
Healthcare recommendations are individualized and depend on factors including clinical assessment, medical and psychiatric history, patient needs, applicable laws, professional standards, and the healthcare professional's clinical judgment.
Summit Mind Psychiatry does not guarantee any particular treatment outcome.
5. Telehealth and Patient Location
Summit Mind Psychiatry may provide healthcare services through telehealth and may offer in-person appointments in Lacey, Washington.
The availability of healthcare services may depend on the patient's physical location at the time services are provided, provider licensure, applicable federal and state law, clinical appropriateness, insurance requirements, and other factors.
Information on this website should not be interpreted as a representation that Summit Mind Psychiatry can provide healthcare services to every individual who accesses the website.
6. Appointment Requests and Scheduling
The website may provide access to appointment scheduling through Summit Mind Psychiatry or third-party scheduling platforms such as Headway.
Submitting or selecting an appointment time does not necessarily guarantee an appointment until scheduling requirements have been completed and the appointment has been confirmed through the applicable scheduling system.
Appointments may be subject to separate policies concerning cancellations, rescheduling, late arrivals, missed appointments, payment, and insurance. Those policies may be provided during scheduling or patient intake.
7. Insurance and Billing
Information regarding insurance participation, billing, or fees is provided for general informational purposes and may change.
Acceptance of an insurance company does not guarantee that Summit Mind Psychiatry participates in every plan, network, product, or benefit offered by that insurer.
Coverage depends on factors including the patient's individual insurance plan, eligibility, benefits, network status, deductibles, copayments, coinsurance, authorization requirements, and other plan-specific provisions.
Patients are encouraged to confirm benefits directly with their insurance plan.
For applicable insurance plans contracted through Headway, Headway may provide billing and claims-processing services. Summit Mind Psychiatry may separately conduct billing for other applicable patients or plans.
Nothing on this website constitutes a guarantee that an insurance company or other payer will cover or reimburse a particular service.
8. Electronic Communications
Summit Mind Psychiatry may provide email addresses, website forms, patient portals, or other electronic methods of communication.
Electronic communications are not continuously monitored and should not be used for emergencies. Although reasonable safeguards are used to protect information, no method of electronic communication can be guaranteed to be completely secure.
Established patients should use designated secure patient communication methods, including the secure patient portal when available, for detailed clinical communications or sensitive health information.
