Summit Mind Psychiatry welcomes referrals from healthcare providers, therapists, counselors, healthcare organizations, schools, and community partners.

We aim to make the referral process straightforward while providing collaborative, patient-centered psychiatric care in a welcoming and affirming environment.

Who We See

Summit Mind Psychiatry provides comprehensive psychiatric evaluation and medication management for children, adolescents, and adults.

Care is individualized, evidence-based, and collaborative, with an emphasis on creating a welcoming and nonjudgmental environment for every patient. We are committed to providing LGBTQIA2S+-affirming care and improving access to psychiatric services for individuals from diverse and underserved communities.

 

Referrals may be appropriate for concerns including:

• Depression
• Anxiety disorders
• ADHD
• Bipolar and other mood disorders
• Sleep concerns
• Medication evaluation
• Psychiatric medication management
• Diagnostic clarification
• Coordination of psychiatric care

Telehealth appointments are available, with in-person appointments available by appointment in Lacey, Washington.

For Therapists & Healthcare Providers

Collaborative Psychiatric Care

We value collaboration with therapists, primary care providers, and other members of a patient's healthcare team.

Patients may continue psychotherapy and other services with their established providers while receiving psychiatric evaluation and medication management through Summit Mind Psychiatry. With appropriate patient authorization, we can communicate with referring providers to support continuity of care, share relevant treatment information, and coordinate recommendations when appropriate.

 

How to Refer a Patient

Healthcare professionals may refer patients to Summit Mind Psychiatry using one of the options below.

Submit referral information through our secure referral system.

Fax a Referral

Referral information and supporting documentation may be sent to our secure fax line.

Secure Fax: 

When available, please include:

• Patient name and date of birth
• Patient phone number and contact information
• Reason for referral
• Relevant diagnoses or clinical concerns
• Current medication list
• Relevant clinical or treatment records
• Insurance information
• Referring provider's name and contact information

Please send only information reasonably necessary to facilitate the referral and coordination of care.

When appropriate, patients may also be directed to our scheduling page to request an appointment.

Insurance & Payment

Insurance coverage and network participation vary by individual plan. For applicable commercial insurance plans, billing and claims processing may be administered through Headway. Summit Mind Psychiatry separately handles billing for certain other insurance programs, including Washington Medicaid/Apple Health and TriWest. Patients should verify their individual eligibility, benefits, copays, deductibles, and network status before receiving services. For current information, please visit our Insurance & Billing page.

What Happens After a Referral?

Once a referral is received, the information will be reviewed to determine whether Summit Mind Psychiatry may be an appropriate fit for the patient's needs. When appropriate, the patient will be contacted regarding scheduling and next steps. Submitting a referral does not establish a provider-patient relationship or guarantee acceptance for treatment. If a patient's needs fall outside the services or level of care provided by Summit Mind Psychiatry, another provider or level of care may be recommended.

Medical Records & Coordination of Care

We believe communication between members of a patient's healthcare team can be an important part of effective psychiatric care.

With appropriate patient authorization, Summit Mind Psychiatry can coordinate with referring providers and exchange relevant treatment information when clinically appropriate. Patients may be asked to complete a Release of Information (ROI) before protected health information is shared with outside individuals or organizations.
